Report a change of circumstances for a cub premises certificate

You must let us know of any change in circumstances. This includes:

• a change of name, club rules and/or registered address of the club (fee applies)
• if you wish to surrender a certificate
• if you require a duplicate copy of a certificate following loss or theft

New applications

You can apply to change your club premises certificate by making a new application. This application needs to be accompanied by the following:

• the current club premises certificate
• the required fee
• a premises operating schedule
